An end of life plan (EOL) is a written document outlining a person’s wishes regarding their care at the EOL. This includes medical, personal and financial decisions. It allows families and healthcare providers to be guided, ensuring your wishes are respected and bringing clarity at a difficult time.

Advanced Care Directive (ACD) documents your wishes for medical intervention including what you do and don’t want if you are unable to make decisions for yourself.

Identify your needs, both current health status and potential future needs.
Discuss your wishes with family, friends and healthcare providers.
Review and update information regularly particularly if circumstances change.
